Its core mission is to "improve your health and well-being," and its Well Connected program seeks to improve com- munity health by engaging the community through education, support and other programs. Focused on the health of the communities it serves, the Well Connected program inspired design and innovation throughout the new building elements, from site planning to building envelope, space planning and interior design to en- hance the overall patient care experience. The new Health and Wellness Center creates a strong sense of place and identity in the community by off ering commu- nity space that regularly welcomes local groups. Moveable glass panels that open to the adjacent gallery enable the facility to accommodate larger community groups for special events. Additionally, food insecurity, a key con- sideration in population health, is addressed in the new center through the creation of a "food pharmacy." The food pharmacy off ers healthful food to its patients and communi- ties to promote healthy living. Exuding neighborly charm A formerly underutilized site was convert- ed into the new community health center. ProMedica envisioned a facility that con- nected to the surrounding neighborhood and was a good neighbor to serve as a com- munity asset and help revitalize the area. Shadow pattern and seasonal changes make the space dynamic. > The exterior landscape extends from the street to interior garden courtyards, providing four-season connections to nature and daylight. Architectural and interior form is distinctive and strongly expresses the ProMedica brand, while conveying a unique sense of place.
